Emergency assistance for Christa Pike was delayed after a failed lethal injection in Tennessee on Sept. 30, according to 911 and dispatch recordings reviewed by The Associated Press.

Pike, 50, received two doses of lethal injection but did not die and was taken to a hospital, the reports said. The recordings show prison officials, emergency dispatchers and responders spent time determining whether a request for help in the execution chamber was legitimate. The first call was logged at 8:40 p.m., and prison officials began allowing responders through the gates about 30 minutes later, according to PBS's AP report.

The reason Pike survived the injections remains unclear. The Tennessee corrections department had not said what it believed caused the failed execution, according to the BBC, which said it had sought comment from the department.
